No, there are no additional eligibility requirements for the one-time payment. However, in order for CDA to issue payment, eligible child care providers must submit a valid W-9 Form.

CDSS has provided CDA with the information for those providers eligible to be paid the one-time payment. These are providers whose local County Human Services Department is unable to provide those payments. Counties include:

Colusa, San Diego, Yolo, El Dorado, Imperial, Madera, Mendocino, Monterey, Riverside, San Bernardino, Stanislaus, Sutter, Tehama, Glenn, Mono, Sierra, Contra Costa, Fresno, Orange, San Mateo, Santa Barbara, Santa Clara, Santa Cruz, Sonoma, Sacramento, and Tulare.

Eligible providers will receive an email invitation from CDA to complete a W-9 Form. Providers can also contact CDA’s Supplemental Rate Payment Program unit to request a copy:

CDA will issue a check for the one-time payment directly to eligible Stage One or Bridge Program child care providers whose valid W-9 Form has been received.

CDA will issue payments no later than November 30, 2023 to eligible providers whose valid W-9 Form has been received.
